    Effective October 1, 2011

    Due to increasing citizen access of Contract Health Services (CHS) programs and significant congressional budgetary cuts from the federal government, the Pokagon Band CHS Dental Program will be limiting dental services in the amount of $1,500 per eligible citizen. In order to meet eligibility criteria, a treatment plan from the citizen’s provider must be submitted to the CHS Program. It will be the citizen’s choice and responsibility to select from the recommended treatment procedure(s), the specific procedures the citizen wishes to complete. 

    Each citizen will have the continued requirement to complete two six month dental maintenance appointments. The appointments will be included in the $1,500 allocation. During the fourth quarter of FY 2012, each additional dental procedure requested by a citizen beyond the $1,500 funding allotment will be reviewed and evaluated by priority. If the necessary funding is available and the dental procedure(s) meets the priority criteria, the cost of the procedure(s) will be covered.
