            The Pokagon Band Department of Health Services is an ambulatory clinic which provides direct patient care in the areas of diagnosis, prevention and treatment of acute and chronic illness, with an emphasis on health and wellness. We serve Native American adults and children, providing primary prevention, screening for and management of chronic disease, general medical care and screening, and support and counseling in the areas of smoking cessation, nutrition and exercise, diabetic management, illness prevention, and more. Our team of dedicated doctors, nurses, and medical assistants, and support staff collaborates with Pharmacy, Behavioral Health, Community Health, Registered Dietician, and Contract Health Services staff to provide the best possible patient treatment and experience.

     

    In-House Program Services include:

     

    • Management of acute and chronic illness
    • Routine medical care
    • Laboratory services
    • Minor procedures
    • Immunizations
    • Wellness visits for adults and children
    • Health and medical education, including illness prevention
    • Periodic health fairs and educational events

     

            When medical needs of our patients are such that they cannot be handled on-site; our medical providers, with the help of our Medical Referral Specialist, will refer for consultation by specialized health service providers.
