Optical Services
Pokagon Band Department of Health Services offers an eye examination and basic eyewear to Pokagon Band citizens and their families, and to enrolled citizens of any federally recognized tribe of American Indian or Alaskan Native. CHS eligible patients, who are up-to-date, do not pay for this service as long as guidelines are followed.
Eligibility guidelines are as follows:
- Everyone who is eligible for the Optical Program must be enrolled with the Department of Health Services (DOHS)
- Pokagon Band Optical Program/Optical covered services are available to all eligible enrolled Pokagon Band citizens who are eligible and enrolled with CHS
- Non-citizen spouses may access the Pokagon Band Optical Program to purchase eyewear. Eyewear will be at full cost, and non-citizen spouses will be solely responsible for payments. Payment will be due prior to delivery of eyewear
- Medicaid recipients, Pokagon Band and other federally recognized tribal children up to the age of eighteen (18) years of age or non-high school graduates are eligible for eyewear through the Optical Program
- Eyewear may be available to American Indian/Alaskan Native enrolled in federally recognized tribes directly through the Pokagon Band Optical Program. All federally recognized tribal citizens must show proof of tribal recognition
