COVID-19 Citizen Vaccination Information

Pokagon Health Services is offering a phased process of COVID-19 Vaccinations to our most vulnerable citizens. As we can roll out what we can for vaccines, we will provide updates to this webpage and also provide updates via text and voicemail when we can. Please make sure your contact information is up to date with the enrollment office. 


4/15/2021 Update - Pokagon Health Services (PHS) has now received the Pfizer COVID-19 vaccine. This vaccine can be given to anyone age 16+, whereas the other vaccine we administer (Moderna) can only be given to people age 18+. Both of these vaccines are two shot series.

At this time, PHS encourages Pokagon citizens age 16+ and anyone they know age 16+ who has not received COVID-19 vaccine but still wants one to call 269-692-5812 to leave a secure message to schedule a vaccination (parents/guardians need to call for individuals that are 16-17 years old). We have Moderna vaccine available for individuals 18+ and Pfizer vaccine available for individuals 16+.

In the event we do not have enough vaccine for everyone who wants it, we will add your name to a waitlist and notify you as soon as we have a vaccine for you; priority is given to Pokagon citizens before the general public.
