COVID-19 Citizen Vaccination Information

Pokagon Health Services is offering a phased process of COVID-19 Vaccinations to our most vulnerable citizens. As we can roll out what we can for vaccines, we will provide updates to this webpage and also provide updates via text and voicemail when we can. Please make sure your contact information is up to date with the enrollment office. 

1/21/2021 Update - Pokagon citizens aged 18-34 years old who have any of the following underlying medical conditions that put them at risk of severe illness from COVID-19 can now call 269-692-5812 to leave a secure message letting us know if you want the vaccine:

  • Cancer
  • Chronic kidney disease
  • COPD
  • Down Syndrome
  • Heart conditions (such as heart failure, coronary artery disease, cardiomyopathies)
  • Weakened immune system from solid organ transplant
  • Obesity (BMI greater than or equal to 30)
  • Sickle cell disease
  • Cigarette smoker
  • Type 2 Diabetes

Depending on the number of Pokagon citizens in this category who elect to receive the vaccine, we may not have enough vaccine for everyone initially. However, we still encourage everyone who meets this criteria to call if they are interested in receiving the vaccine as we will continue to contact people as vaccines become available. We contact people in the order we receive their voicemails.
