COVID-19 Citizen Vaccination Information

Pokagon Health Services is offering a phased process of COVID-19 Vaccinations. As we can roll out what we can for vaccines, we will provide updates to this webpage and also provide updates via text and voicemail when we can. Please make sure your contact information is up to date with the enrollment office. 

 

COVID-19 Vaccines

PHS continues to offer the initial COVID-19 vaccine. If you have questions about the COVID-19 vaccines, please reach out to your Primary Care Provider or find more information on http://cdc.gov/coronavirus.

Children aged 5 – 11 (November 5, 2021):

Pfizer’s vaccine for kids has been authorized to be administered to children aged 5 – 11.  The FDA announced on October 29, 2021 that based on a thorough evaluation of the data, the vaccine was 90.6% effective in preventing COVID-19 in children ages 5 – 11 and no serious side-effects have been detected in its ongoing study. This is a 2-dose series, with the second dose given 3 weeks after the initial one. PHS expects an order of Pfizer for kids to arrive by Tuesday November, 9 and has appointments available starting at 5 p.m. on Tuesday. Boosters (November 22, 2021):

COVID-19 Boosters for Moderna and limited amounts of Pfizer are available at Pokagon Health Services (PHS) six months after your initial series. The CDC has expanded eligibility requirements for boosters to the following groups:

  • Ages 50 years and older or those 18 and older who live in a long-term care setting should get a booster
  • Ages 18 years and older may get a booster

If you are eligible for a COVID-19 booster, please call the PHS Vaccine Line on (269) 692-5812 and leave a message to request an appointment. We expect an influx of calls currently, so please be patient as you wait for one of our team to return your call. Appointments will be scheduled on a first come first served basis.

J&J COVID-19 Boosters have been recommended for 2 months after your initial dose for all those 18 and older. PHS does not currently carry supplies of the J&J vaccine but can refer you to your local health department.

 

Third Doses:

Certain individuals who have moderately to severely compromised immune systems may be eligible to receive a third dose of the vaccine.  This is intended to improve the vaccine response to the initial series.  A third dose of an mRNA vaccine must be given at least 28 days after the second dose.  Please contact our vaccine voicemail at (269) 692-5812 if a 3rd dose is recommended for you. 

 

Children aged 12 – 17:

Pfizer is the only vaccine approved for ages 12 – 17.  PHS does not have regular access to Pfizer vaccines due to the method of distribution from our supplier, and therefore you may be directed to you to your local health department for Pfizer vaccines.

Flu Vaccines

Adult Influenza Vaccines are also now available through our vaccine clinic starting September 20 and Pediatric Influenza Vaccines will be available October 4.  Flu vaccines can be given at the same time as your COVID-19 vaccine.  Please schedule through our vaccine line on (269) 692-5812.
