COVID-19 Citizen Vaccination Information

Pokagon Health Services is offering a phased process of COVID-19 Vaccinations. As we can roll out what we can for vaccines, we will provide updates to this webpage and also provide updates via text and voicemail when we can. Please make sure your contact information is up to date with the enrollment office. 

 

COVID-19 Vaccine Update 9/01/2021

Moderna vaccines are currently available at PHS for all 18-year old’s and up. Please call our vaccine voicemail on 269-692-5812 to leave a message and our vaccine team will contact you to schedule an appointment.

Children aged 12 – 17:

Pfizer is the only vaccine approved for ages 12 – 17. PHS currently has limited access to Pfizer vaccines due to the method of distribution from our supplier, and therefore, if supplies are not available, you may be directed to you to your local health department for Pfizer vaccines.

Local Health Departments

3RD Doses:

Certain individuals who have moderately to severely compromised immune systems may be eligible to receive a 3rd dose of the vaccine. This is intended to improve the vaccine response to the initial series. A 3rd dose of an mRNA vaccine must be given at least 28 days after the 2nd dose. Please contact our vaccine voicemail on 269-692-5812 if a 3rd dose is recommended for you. 

Boosters:

Boosters are NOT currently approved for those who have already been fully vaccinated. If and when boosters are approved, PHS will be sharing information on how to schedule your booster shot. Until then, please watch this space.

Vaccine Cards:

If you have lost your vaccine card and would like a replacement, please leave a message with your name, date of birth and the request for a replacement on our vaccine voicemail at 269-692-5812.
