Important Information for November and December Walmart Grocery Card Program Applications.

December 10 at 5 p.m. is the December Deadline.
December 10, 2021 at 5 p.m. is now the deadline for December Walmart Grocery Cards and COVID-19 Emergency Assistance Program (EAP) applications.

December applications for the Walmart Grocery Card and for the COVID-19 EAP will close December 10, 2021. All materials needed to complete the applications must be received no later than 5 p.m. on December 10, 2021.

Applications for Both November and December Will be Taken in November.
In order to avoid missing the December 10, 2021 deadline, applicants may apply for both November and December assistance when applying for the programs in November.

If you already applied for either program on or before November 8, 2021, you must still apply for December assistance. You may apply for December assistance in November.

The Program

The Pokagon Band Tribal Council has approved the 2021 grocery card program (“Program”) to assist tribal households with necessary and reasonable increased expenses (“Increased Need”) caused by the COVID-19 emergency (“Emergency”).

  1. The Program will be provided each month as determined by available funding.

  2. The application for Assistance (“Application”) provided by the Band must be completed and submitted for each month of eligibility.

  3. The deadline to submit a completed Application is the last day of the month of Assistance at 5 p.m.


Program Eligibility

  • The applicant ("Applicant") shall have the burden of proving eligibility under the Program.

  • Program assistance (“Assistance”) shall not duplicate any other assistance, including that provided by the Pokagon Band under other COVID-19 relief programs or under any federal program.   The Increased Need must be over and above the needs already met by other assistance programs.

  • At the time of application, an applicant (“Applicant”) must be:

  • (a) A Pokagon Band citizen (“Citizen”) that is at least 18 years of age; or
    (b) A non-Citizen parent (including biological, adoptive, and step-parent) who has sole or joint custody (both legal and physical) of a Citizen who is less than 18 years of age ("Non-Citizen Parent"); or
    (c) A non-Citizen guardian who has sole or joint custody (both legal and physical) of a Citizen who is less than 18 years of age ("Non-Citizen Guardians"); and
    (d) The head of household listed in CRM.
    A $150 grocery card will be provided for each named person in the household, provided that:

  • The Applicant may apply for only as many grocery cards as he/she is able to document Increased Need; and

  • Each named person in the household shall receive only one grocery card, regardless of how many applications his/her name appears on; and

  • The persons in the household listed on the application shall be verified by the Pokagon Band Department of Social Services, and only verified persons in the household shall receive assistance under this Program.

  • The Applicant is responsible for being able to document the increased need arising from increased costs related to the Emergency. The Applicant must produce such documentation upon request.

  • Any decision by the Pokagon Band regarding the Program, including, without limitation, the Application or the Assistance, shall be final and shall not be subject to review or appeal.

  • Nothing in this Program shall: (1) be construed to vest in any person any right or interest in any Band revenue or assets; (2) create any obligation that is legally enforceable against the Band; or (3) waive the sovereign immunity of the Band or any of its officials or employees.

  • The Tribal Council reserves the right to amend and/or repeal this Program, subject to applicable law.

  1. Information contained in the Application is confidential and will only be used for official tribal and regulatory purposes, which may include audits by the Pokagon Band, by the Office of the Inspector General, and by the IRS.
